Notice of Jiangsu Provincial Health and Wellness Committee on Disease Prevention and Control on Adjusting the Vaccination Scheme of Polio (Su Wei CDC [2022] No.51)

Municipal Health and Health Committees and Provincial Centers for Disease Control and Prevention:

In order to maintain the polio-free status in our province and prevent the risks of vaccine-associated paralytic poliovirus (VAPP) and circulating vaccine-derived poliovirus (cVDPV), according to the World Health Organization (WHO) and China’s polio eradication strategic plan, combined with the actual situation in our province, and with the approval of the provincial government, it was decided to add the current "2 doses of inactivated polio vaccine (IPV) and 2 doses of bivalent oral attenuated polio vaccine" from January 1, 2023. In order to do a good job in the adjustment of vaccination programs, the relevant matters are hereby notified as follows:

First, strengthen organizational leadership and clarify departmental responsibilities.

All localities should deeply understand the close relationship between vaccines and people’s health, and fully understand the importance of adjusting the immunization program of polio vaccine. Health departments at all levels should take the initiative to coordinate local finance, drug supervision and other departments, establish a departmental coordination mechanism with clear responsibilities and efficient operation, and make every effort to ensure the smooth adjustment of immunization programs. According to the Implementation Plan of Polio Vaccination in Jiangsu Province (Annex 1), the local vaccination implementation plan should be formulated in time, and the financial department should coordinate the purchase funds of syringes required for polio inactivated vaccine (IPV) vaccination and the funds needed for vaccination, and cooperate with the drug regulatory authorities to do a good job in quality supervision and management of the circulation of polio inactivated vaccine (IPV), so as to form a good working situation of joint management, close cooperation and seamless connection, and jointly implement all measures.

Second, do a good job in transition to ensure policy convergence.

All localities should do a good job in the transition and preparations for the adjustment of polio vaccination programs. Health departments at all levels shall, according to the adjusted vaccination plan, timely carry out training for vaccination units and staff within their respective jurisdictions, and standardize relevant vaccination operations and processes. At the same time, in conjunction with the drug regulatory authorities, strengthen the supervision of vaccine circulation and use, and make every effort to ensure safe and effective vaccination. The Provincial Center for Disease Control and Prevention should do a good job in the procurement, distribution and supply of polio vaccine, and the disease control institutions and inoculation units of districts, cities and counties (cities, districts) should do a good job in vaccination, disease monitoring and abnormal reaction monitoring of suspected vaccination to ensure that the polio vaccination rate is maintained at a high level.

Third, strengthen popular science propaganda and carry out health promotion

All localities should organize health education and health promotion, and actively publicize the relevant popular science knowledge about the adjustment of polio vaccination program. Through television, radio, internet, parents’ classroom, media and other flexible forms, parents or their guardians of children can fully understand the significance of the adjustment of polio vaccination program. It is necessary to pay special attention to the propaganda in areas with weak immunity and floating children to ensure that the target population reaches a high awareness rate. It is necessary to do a good job in risk communication and response, strengthen the monitoring of relevant emergencies and public opinions, respond to social concerns in a timely manner, gain the support of the masses for vaccination work, build a positive social publicity environment, and form a good social public opinion atmosphere.

Fourth, organize research and guidance to solve problems in time.

Health and health departments at all levels should, jointly with the financial and drug supervision departments, strengthen research and guidance on the adjustment of local polio vaccination programs, urge all relevant units to earnestly implement various policies and measures, find and coordinate problems in a timely manner, and report relevant information to our Committee in a timely manner.

Implementation plan of polio vaccination in Jiangsu Province

In order to keep polio-free in our province and prevent the risks of vaccine-associated paralytic poliomyelitis (VAPP) and circulating vaccine-derived poliovirus (cVDPV), the routine immunization program of polio vaccine for children in our province was changed from "2 doses of inactivated polio vaccine (IPV)+ 2 doses of live attenuated oral polio vaccine (bOPV)" to "4 doses of inactivated polio vaccine (IPV)". In order to guide all localities to do a good job in the adjustment of the polio vaccination program, the "Implementation Plan for Polio Vaccination in Jiangsu Province" is formulated, with the specific contents as follows:

I. Implementation Time

Since January 1, 2023, the routine immunization of polio vaccine for school-age children in our province has been adjusted from "2 doses of IPV+2 doses of bOPV" to "4 doses of IPV".

Second, immunization procedures and vaccination methods

(1) Immunization procedures

1. Children born after November 1, 2022 (inclusive) should be vaccinated according to the adjusted "four doses of IPV" immunization program. Children should be vaccinated with one dose of IPV basic immunization at the age of 2, 3 and 4 months, and one dose of IPV booster immunization at the age of 18 months.

2. Children born before November 1, 2022 shall be vaccinated according to the "2 doses of IPV+2 doses of bOPV" immunization program before adjustment.

(B) IPV vaccination methods

Inoculation dose was 0.5ml, intramuscular injection.

Third, the principle of vaccination

1. In accordance with the principle of "classified implementation, informed consent and residence management", free polio vaccine is given to school-age children who meet the immunization procedures.

2. Before vaccination, the vaccination personnel should ask about the health status of the recipients and whether there are any vaccination taboos, inform the guardians of the recipients of the vaccine varieties, functions, taboos, adverse reactions and on-site observation and other matters needing attention, and truthfully record the notification and inquiry.

3. If the guardian of the recipient voluntarily requests to vaccinate the non-national immunization program vaccine containing IPV components, if the child has completed the whole vaccination of 4 doses of combined vaccine containing IPV components according to the vaccine instructions, it can be regarded as a substitute with the same effect and complete the whole vaccination of the immunization program vaccine.

4. When the follow-up vaccination cannot be completed with the same vaccine from the same manufacturer (including replanting), the follow-up vaccination can be completed with the same vaccine from different manufacturers.

5. For children with late or missed vaccination of polio vaccine, it is enough to replant the corresponding doses, and it is not necessary to start the whole vaccination again.

Fourth, organize the implementation

(A) Division of responsibilities

Polio vaccination work should be under the leadership of health departments at all levels, and vaccination clinics should be responsible for the specific implementation of vaccination within their respective jurisdictions, including informed notification by the recipients and their guardians, vaccination operations according to norms, and information registration, adverse reaction monitoring and data summary and reporting should be done well. Disease control institutions at all levels are responsible for organizing the implementation and technical guidance of vaccination within their respective jurisdictions, implementing the vaccination rate monitoring, adverse reaction monitoring, effect evaluation and data summary and reporting after the adjustment of polio vaccine immunization strategy, and the provincial CDC will do a good job in the procurement, distribution and supply of polio vaccine.

(2) Personnel training

Disease prevention and control institutions at all levels and vaccination clinic staff should carry out multi-level, high-density, full-coverage polio vaccine immunization program adjustment training, including vaccination implementation, vaccine and cold chain management, adverse reaction monitoring and disposal, risk communication, etc., and increase the corresponding content according to the actual work in various places.

(3) Standardize inoculation

All vaccination clinics should master the taboo, caution and precautions in the vaccination instructions in accordance with the requirements of "Guidelines for Vaccination Work" and "Operation Manual for Vaccination in Jiangsu Province", do a good job in health inquiry and notification before vaccination, implement the "three checks and seven to one verification" system, and scan the code for vaccination, so as to ensure the traceability of the whole vaccination process and avoid vaccination errors and accidents.

(D) Vaccine and cold chain management

Disease prevention and control institutions at all levels and vaccination clinics are stored and transported in strict accordance with the storage conditions on the polio vaccine instructions. When receiving polio vaccine, we should check the batch number and quantity of the vaccine and confirm that it is consistent with the batch number and quantity on the delivery unit’s outbound order. At the same time, we should register the inbound and outbound documents and enter them into the information system according to the relevant requirements of Jiangsu province’s immunization program product management. Immunization planning polio vaccine and non-immunization planning vaccine containing polio components should be registered and stored separately, and the storage place should be clearly marked. Maintain the cold chain equipment for storage and transportation of polio vaccine according to the requirements of "vaccination work specification".

V. Monitoring and handling of suspected adverse reaction to vaccination (AEFI)

Vaccination clinics should do a good job of on-site observation after vaccination, and immediately organize emergency rescue and other work for acute severe allergic reactions during observation, and do a good job in monitoring AEFI according to the National Monitoring Plan for Suspected Adverse Reactions of Vaccination, and cooperate with disease prevention and control institutions to carry out AEFI investigations. Disease prevention and control institutions at all levels should do a good job in AEFI case reporting, verification, investigation and data collection according to the requirements of the monitoring plan. For AEFI that needs to be investigated and diagnosed, an expert group for investigation and diagnosis of abnormal reaction of vaccination should be organized in time in accordance with the requirements of "Work Specification for Investigation and Diagnosis of Abnormal Reaction of Vaccination in Jiangsu Province".

VI. Publicity and Media Communication

All localities should carry out publicity and media communication according to the actual situation of this plan and work, focus on the main purpose and significance of immunization strategy adjustment, issue popular science materials, carry out public opinion monitoring and research, strengthen the guidance of media public opinion when necessary, and convey correct information to the public. At the same time, strengthen the skills training of vaccination staff in publicity and communication, scientifically answer public concerns and do a good job in implementation.
